"Screening Mammograms and Additional Breast Health Services for Uninsured and Underserved Population"

Description: The Central Georgia Breast Care Center provides mammograms and breast health services to the uninsured and underinsured population in a seven- county region of Central Georgia, which includes the rural counties of  Baldwin, Bibb, Crawford, Jones, Monroe, Peach and Twiggs. The goal of this partnership is the early detection of breast cancer and the improvement of life expectancy and quality of life.

First Choice Primary Care

"Breast Cancer Screening for the Underserved"

Description: First Choice Primary Care is a Federally Qualified Health Center providing high quality accessible primary care to the medically underserved of Bibb County and surrounding areas. Their goal is to provide comprehensive breast health services, including breast health education, clinical exams and mammography screening services, to medically underserved women. Their current patient population is predominantly African American, and includes other underserved groups such as homeless and Hispanic women who frequently face significant barriers to care.

"Regional Outreach Project for Breast Health"

Description: The purpose of the Central Georgia Regional Outreach Project is to provide mammograms for lower income, uninsured women which includes many women of color. The target group includes women, who without this assistance, are unable to afford a mammogram and navigate the health care system. A Registered Nurse serves as a Nurse Care Manager and coordinates the activities related to education, obtaining a voucher for a mammogram, scheduling the screening and follow up as needed. The project's focus is to provide women who are 200 percent of the poverty level breast health services.

"Girls Night Out for Good Health"

Description: The purpose of Girls Night Out for Good Health is to improve screening mammography outreach by providing after hours mammograms in a VIP, spa like atmosphere. The target population is women who are unable to obtain mammograms during the regular business hours of 8am-5pm, women past due for annual screenings, and women receiving mammograms for the first time. The project goal is to increase the number of women obtaining mammograms by offering additional service times in a relaxing environment. In addition to obtaining a mammogram, manicures, facials, and massages are provided.

Oconee Regional Medical Center, Inc.

"Focused on the Fight, Focused on You"

Description: The goal of this program is to provide free and/or reduced cost outpatient breast screening and diagnostic services to women residing in Baldwin County, especially medically underserved populations including minority and migrant populations. ORMC will be partnering with the Baldwin County Health Department, in addition to local qualified health care providers. The Baldwin County Health Department will ensure such patients meet the referral guidelines for receipt of the free or low cost mammography and refer such individuals so identified to ORMC. Further, if the patient has no means of physical transportation, ORMC Non-Emergency Transport (NET) will coordinate with the patient and the Health Department to arrange for transportation to their appointment under the grant funding.

"Cancer Well-Fit"

Description: The Cancer Well-Fit Project is a free 12-week oncology recovery program for breast cancer patients. The program combines exercise, education, nutrition, mind/body activities, and support to assist people with cancer in returning to their fullest physical and emotional potentials. The holistic approach incorporates all aspects of a person's health and well-being. When a woman is faced with this life threatening illness, the Cancer Well-Fit team brings these elements together to aid recovery, maintain wellness, and enrich the quality of life.

The 12-week program meets 3 times per week on Mondays, Wednesdays, and Fridays from 9:00-11:00 am.
Each two-hour session includes exercise followed by education on coping skills, stress management, pain management, complementary/alternative medicine and group support.

Unlimited Potential Community Development Center

"Cancer Prevention Awareness Coalition - CPAC"

Description: Unlimited Potential CDC is implementing preventative, wellness programs through community-based educational seminars, weight-management programs and strategic alliances with health and fitness professionals and testing facilities. They aim to provide mammogram vouchers as well as preventative wellness testing including blood testing and body fat analysis.This program looks at the behavioral and lifestyle changes that women can make to help reduce their risk of breast cancer along with other diseases such as diabetes. The program targets low income, uninsured populations, some of which are homeless or in transitional housing.

No physical address - seminars held in different locations every month.
